*** The Newborn Arrives ***

"It is a boy!" shouts the obstetrician excitedly. "He looks great. He's got all his parts. As a matter of fact, he just wet on me. He is big. He weighs a little over nine pounds!"

New mom, happy and relieved replies, "But you told me last month that it was to be a girl!"

M.D. replies, "You must have misunderstood me." Actually she hadn't. Many OB's will tell their pregnant patients they will have a boy; then write on the chart: GIRL. They are right just about 50 percent of the time.

** "Do you Have a Pediatrician?" asks this OB. **

This OB has a few choices of pediatricians.

* Some baby doctors are very serious, nit-picking, relying heavily on the laboratory. * Your OB also knows a few who are supportive, liberal, intelligent, and willing to talk and be supportive with the parents' wishes about child rearing and treatment. * These two groups constitute the doing doctors and the talking doctors. The OB can often steer the parents to the doctor with the best fit.

** The Pediatrician Sees The Baby For The First Time **

The pediatrician is alerted to the arrival of his new patient. He makes rounds the next morning and sees baby Alex (for Alexander, formerly know as Alexandria.) He has been in practice for eight years now and pretty well knows the routine.

* He scans Alex's skin for birthmarks, polyps, milia, brown pigment deposits, stork bites, and extra fingers and toes.

* Before Alex starts to cry excessively, he listens to Alex's heart and lungs. All okay there.

* He palpates Alex's abdomen and checks the umbilical stump. All okay there.

* He feels for the femoral pulses. (Coarctation of the aorta; okay there.)

* He holds Alex prone in his palm and runs a finger up his spine to see if he arches his back --- he does.

* He then holds Alex up by his heels and notes his straight spine and that there is no deviation of his head to one side or the other.

* He holds Alex upright and puts his feet onto the examining table and notes if Alex steps; he does.

* He checks the Moro reflex.

* He then feels Alex's eyeballs through the closed lids for glaucoma. Okay there.

* His final test: he lubricates his little finger and with Alex lying supine on his back, the doctor slowly inserts it into Alex/s rectum no more than one inch. He notes a tightness about his finger; the anal ring. Not a stenosis, but enough to cause some colic or intestinal distress. It is like the iris of a camera. It is the area where the bud from the intestines meets the dimple from the ectoderm. (Later on Alex's mother will comment: " Ever since you examined Alex he seems more comfortable.")
