*** On Handling Problems With Babies ***

The kindly, friendly, knowledgeable pediatrician gets to his office and finds his first patient of the day is waiting, and he already has four phone calls waiting for him. Mrs. Jones name has a star next to it. He dials.
** Why is my baby fussy? **
Mrs. Jones: Doctor, I am not a nervous person but every time I pick my baby up, he cries. He is three weeks old and since birth he has been fussy during feedings, especially when I move him from one breast to the other. I'm eating bland foods. Should I put him on the bottle? Is it hunger? I saw another doctor and he thought I should supplement the breast with the bottle. He recommended a sedative. I don't want to do that.
Pediatrician: What was his birth weight?
Mrs. Jones: He was big: nine pounds.
Pediatrician: Sometimes when a baby is big, he cannot get enough to completely assuage hunger, but I have another idea. Feel his upper chest where his collarbones are. See if you can feel a knob on one of the clavicles.
Mrs. Jones: She feels the collar bones on Jimmy. Yes there is a lump on the left one.
Pediatrician: It is not serious. The collarbone is the easiest one to break, and the quickest one to heal. It will heal by itself. The lump is a blood clot that forms at the fracture sight, and, as the bone heals the swelling will disappear in about two to three months. It must be the reason why he cries when you lift or move him. We could put on an elastic bandage to hold his shoulder back and keep the bone ends from grating together. Or you could nurse him with him lying supine in your bed or on the floor and you would lower yourself down on to him so his lips touch your nipple. If he seems to be in more pain, come in and I will fix this elastic bandage to hold that shoulder back and immobilize the clavicle while it heals. You might give him some vitamin D for a while.
* Why does my baby have a crack at the corner of his mouth? *
Phone call from Alex's mother: I know I'm coming to the office in a couple of weeks but I thought I had better ask about this now. He has a crack at the corner of his mouth and redness on the skin about his anal opening. Is this serious? What is the problem?
Pediatrician: If a rash shows up on Tuesday, it is most likely something you ate on Sunday or Monday. Tomato? Orange juice? Chocolate? Maybe you started some vitamin Drops?
Alex's mother: We did go out to a pizza place on Sunday. I was dying to get out of the house after being a good mother for two weeks. The pizza had everything from tomato to anchovies to cheese to pepperoni and peppers.
Pediatrician: Wow. It could have been one or all of them together. And, I suppose, you had some chocolate for dessert. "Eat it today and wear it tomorrow" is the rule. Just put some bland ointment like lanolin or Calendula, or open a vitamin E capsule, or use something with Aloe Vera in it. They would be safe. Don't use any crème with cortisone in it frequently, as it tends to make the skin atrophy.
* Why is my baby's face covered with white pimples?
Another patient calls: My baby is three weeks old and her face looks like an adolescent with white pimples just where the oil glands are. What is that?
Pediatrician: Yes, it may be a harbinger of what might happen in 13 years. It is related to diet and hormones from you. Leave it alone; it will disappear in just a week or so and return when she is older. I have seen these pimply rashes in babies if the mothers had eaten peanut butter or a tuna sandwich before delivery. Leave them alone.
