*Possible Causes and Treatment For Nosebleeds*
(prepared from content provided by Dr. Lendon Smith)
Dear Dr. Smith, I am getting mixed messages from family
authorities about nosebleeds. Some say lie down with the head
back. Others say just sit and be quiet. Why does my
seven-year-old daughter get so many? Sincerely, Mrs. Epistaxis, East River, CO
Dear Mrs. Epistaxis, You have asked an expert. I have suffered
from these all my life. My Dad, the pediatrician, sent me to his
ENT friend who would cauterize (after a local anesthesia) my
nasal septum with a hot needle to scar the capillaries there. It
worked for a few days, but the red drip always returned. I
learned to control them by avoiding fights, excitement and
straining at BM time. Mom instructed me to "oil my nose" as part
of the bedtime ritual. That was to keep the dry area from cracking and bleeding again.
The best results I had when I felt the blood pouring out was to
take a piece of tissue or cotton, wet it, and push it up against
the septum. Then I would pinch the nostril tight, as most
nosebleeds come from the last one-inch of the septum. I would
lean forward not backward this would allow me to tell if the
bleeding had stopped. Lying supine permitted the blood to flow
down to my stomach and then I would vomit, starting the whole mess again.
I squeezed my nose for ten minutes -- by the clock --- and then
leave the pledget in place for another ten minutes. Then I would
pull this out slowly. A clot of blood on the surface indicated
that my coagulation ability was active. Then I would poke some lubricant,
like petroleum jelly, into the eroded area, and tiptoe around for a few days.
One of our sons was a nose bleeder and we finally realized it was
a dairy sensitivity. He had colic, ear infections, headaches, and
bed wetting all from dairy products. No dairy products no
problems. So I noted that I had the same problems after cheese,
milk, or ice cream. Dairy products are also constipating to most
people. It could be that your daughter is straining to have a
movement, and that extra pressure starts the bleeding. Your dry
climate might have something to do with this irritating condition.
Treatment: Stop all dairy products, grease her nose at bedtime
with some lotion, and make sure she is getting enough roughage.
If all else fails, you might have the ENT doctor take a look.
Keep in touch, Lendon Smith, M.D.
